Financial Aid Forms and Worksheets
Begin the financial aid process each year by completing completing your FAFSA application to calculate your financial need for college. The FAFSA is the application used to apply for federal, state, and University of Northwestern financial aid.

How to submit forms
Send completed forms to the University of Northwestern Financial Aid Office via:

If your FAFSA is selected for verification by the Department of Education, you will need to complete a Verification Worksheet and follow the instructions explained on the Worksheet for income verification.
If you were selected for verification for verification for 2023-24, and your verification group was either V1-D or V1-I, please use our secure portal, Dynamic Forms, to complete the Verification Worksheet and upload any documents requested. You can find instructions on how to complete the Verification Worksheet for a dependent student here. To get started please choose the appropriate Verification Worksheet for the 2023-24 school year:

If your verification group is V4 or V5, please download and print only one of the verification worksheets below. You will need to complete your Verification Worksheet in the Financial Aid Office. If you are not sure of your verification group, please refer to the verification letter that we sent you, or contact us at 651-631-5212.
Parent and Student Appeal Forms
The Department of Education gives us limited authority to make adjustments to a student's eligibility when circumstances arise that are out of the control of the parent/student. If a parent/student feels that they have an appealable situation, this form should be completed and returned to the Financial Aid Office. Please note that the parent and student appeal processes are only available to students who have a completed FAFSA for the current academic year, are receiving need-based aid, and have an EFC of greater than zero. If you are unsure whether you are eligible for an appeal, please reach out to the Financial Aid Office.
Traditional Undergraduate Students
Study Abroad and Off-Campus Study
This is only for students seeking a traditional undergraduate degree.
Students may be considered for financial assistance for full-time study abroad or off-campus study. In order to be considered for financial assistance through University of Northwestern, the program must be recognized by the university, the student must have established at least one semester of enrollment on campus as a matriculated student and the student must meet program qualifications.
There are specific policies followed for awarding of aid for study abroad and off-campus study. Students must fill out the Off-Campus Assistance Form for evaluation of eligibility. Students should expect that institutional aid will be treated differently for the semester(s) abroad or off-campus.
Some students will also need to have a Consortium Agreement completed. Summer semester studies are considered at the end of a school year. Submission of these forms does not guarantee assistance is available and are for students seeking a traditional undergraduate degree.
During the 2023-24 school year, students taking a combination of Northwestern courses and courses from a state/community college/university should complete the 2023-24 Off-Campus Studies Assistance Form: Combination of Credits to be considered for financial aid.
